Click on the above link to register. Click on file a claim button and then register button is on the top right.

 

Once logged in, click on Claims Center. From there, simply click the File an Online claim button to file many types of claims such as: Wellness, Doctor’s Office Visits, Accident,  Sickness, etc etc. You may also download claim forms and find answers to many of your claims related questions.
