***The Gun Shot Wound Policy must be enrolled with an Agent.

Everything else may be self-enrolled***

1. Connect to the online enrollment system through your web browser at:

2. Enter the username and password by following the schema below:

USER NAME: ANA5Q5X- +Employee ID
PASSWORD: First Initial + Last Name + 4-Digit Year of Birth

EXAMPLE LOGIN:
Employee Name: John Smith
Employee ID: 123456
USERNAME: ANA5Q5X-123456
PASSWORD: JSmith1985

3. You will be prompted to reset your password and create a security question.

scroll to the bottom of the page and enter your email address and click the " I agree"

button.

Your password will be used as your electronic signature so please write it down

so you won't forget.

4. The next page is the welcome screen. Scroll to the bottom right-hand side of the screen and press the " enter my enrollment" button.

5. Update and verify your employee profile. The disability is based off your gross income so make sure you put in the correct number.

6. Review your member benefits and enroll.
